What to do when the NDIS participant passes away? Supports Through Difficult Times.

NDIS acknowledges the significance of providing support to families, caregivers, and networks when a participant passes away. Our dedicated Support Team is trained to offer compassionate and efficient assistance during this challenging period.

How to Notify the NDIS?

Informing NDIS of a participant’s passing is important, and anyone can notify the NDIS. You can do so by:

Call the National Contact Centre (NCC) at 1800 800 110.

Completing the “Notify the NDIA When a Participant Dies” form and submitting it via email, mail, or in person.

What to Provide?

When notifying the NDIA, essential details such as the participant’s name, address, date of birth, and NDIS number are needed.

Response

Upon notification, NDIA Support Team will promptly reach out to you. The team will provide sensitive and empathetic assistance during the difficult time along with updating the documentations. This includes discontinuing funded support and services.

How does it Work?

The NDIS commission aims to solve the issue over a convenient phone call. If needed, you can request an alternative time for the discussion. In certain situations, additional information may be required to confirm the participant’s passing through a clear communication.
